For practices on CMS’s ASM participant list

ASM functional status surveys,
completed by phone.

The Ambulatory Specialty Model starts January 1, 2027. A patient-reported functional status survey sits inside a score that adjusts your Medicare Part B payments. Our voice agent collects it in a normal conversation, with retries, and returns structured results before the visit.

01 / The conversation

A human conversation.
A useful record.

Listen to the moment a routine survey reminder becomes a chance to get the details right.

01 A patient flags an incorrect email.

02 The agent asks, then reads it back.

03 The next step is recorded for the team.

51 seconds. One small correction.
One less loose end for the front office.

02 / What changes in January

A score that moves
Part B payments.

The Ambulatory Specialty Model starts January 1, 2027. Quality and cost carry equal weight in the final score, and a patient-reported functional status survey sits inside the quality half.

±9% 1

Range of the Medicare Part B payment adjustment in the first ASM payment year. Every participant carries it.

+5 pts 2

Proposed quality-score bonus for submitting baseline and follow-up patient-reported outcome data for at least 20 patients.

+10 pts 2

Final-score adjustment for participants in small practices; +15 points for solo practitioners.

03 / Where the phone call fits

Four requirements.
One conversation each.

The measures ask for a validated survey on file, some of them twice. Getting it back from the patient is the part that lands on your team.

Low back pain cohort

Functional Outcome Assessment

MIPS Quality ID 182 2 4

Proposed Proposed for 2027, replacing the retiring Functional Status Change measure (Quality ID 220).

What it asks for

A standardized functional outcome tool documented on the visit date or within the prior 30 days, and a care plan when deficiencies are found. Tools the measure accepts include the Oswestry Disability Index, Roland-Morris and PROMIS.

Where the call fits

We call before the visit. The score is in the chart when the patient walks in; your clinician documents the plan.

Heart failure cohort

Functional Status Assessments for Heart Failure

MIPS Quality ID 377 2 4

In the heart failure quality measure set.

What it asks for

An initial and a follow-up patient-reported functional status assessment for each heart failure patient, with a tool such as the KCCQ, MLHFQ or PROMIS-Plus-HF.

Where the call fits

Two calls, months apart, without asking the patient to come back for a second visit.

Both cohorts

Voluntary patient-reported outcome data submission

Up to 5 additional quality points 2

Proposed Proposed for 2027. CMS says it intends to adopt a PRO performance measure later; this is the data it will be built from.

What it asks for

Baseline PRO data for at least 20 patients, then follow-up data for at least 20 of them in the next collection period. CMS anticipates at least six months between the two.

Where the call fits

Baseline and follow-up go on the calendar, not on the front desk. The patient never has to log into anything.

Both cohorts

Data completeness

75% of eligible patients 2

Finalized in the CY 2026 rule.

What it asks for

Quality data for at least 75% of the patients who meet each measure’s denominator, regardless of payer.

Where the call fits

Completion is the whole game. Retries, voicemail handling and the patient’s preferred language are built into every call.

What we administer: HOOS JR., KOOS JR., and PROMIS Global-10 today; additional validated instruments on request. Which tool satisfies a measure is set by the measure specification, not by us.

04 / A simple handoff

Your workflow.
With follow-through built in.

Fits alongside your EHR.
Works around your patients.
Stays out of your team’s way.

01

Start with your patient list.

Upload the patients who are due for a survey. A simple list gets each one into the right outreach window, whatever program you report into.

No integration project to get started.
02

We make the conversation happen.

Your agent calls in the patient’s preferred language, with follow-up windows, retries and voicemail handling.

03

Less paperwork. More time for patients.

Help your team work more efficiently with structured responses, a completion dashboard and exports that fit your reporting workflow.

From patient conversation to audit-ready data.

05 / The evidence

Built on evidence.
Open to scrutiny.

From A/B testing to patient-reported outcomes research, we look beyond the call to the quality of the answer.

Questions about the model

Straight answers
on ASM.

How do I know if we’re in ASM?

CMS published a preliminary participant list in January 2026, by NPI, covering clinicians in the selected areas who treated at least 20 Original Medicare patients with low back pain or heart failure in a year. Participation is mandatory for clinicians on the list.

CMS participant dataset
What if the final rule changes in November?

Everything marked “proposed” on this page comes from the CY 2027 proposed rule. We update this page when the final rule publishes. The workflow is the same whichever functional status tool the final rule names.

Do you submit the data to CMS for us?

No. We return structured, per-patient results and exports; submission runs through your existing registry, QCDR or EHR reporting workflow.

Which instruments do you administer?

HOOS JR., KOOS JR., and PROMIS Global-10 today; additional validated instruments on request. Which tool satisfies a measure is set by the measure specification, not by us.

We’re a pain, PM&R, spine or cardiology practice, not orthopedics. Does this apply?

Yes. The agent runs the same conversation for any validated instrument, and the low back pain and heart failure cohorts are exactly who this page is for.
